I did a little looking to see if I could help you. From what little I have read the forms are no different than one from the United States, but there may be some difference in how they are 'filed'. I know that they have to be signed in front of a notary to be official here and would imagine that the same thing needs to be done there.. though maybe it isn't called a notary in Canada. Anywho, you didn't specify much about what kind you were looking for; so, I found a few to offer.
